Eligibility & Enrollment
At Daydreams-ADCC, we welcome adults 18 years of age or older who require supervision for daily living skills, are looking to make new friends, and wish to participate in various recreational and community activities. Our center is designed to benefit those who can thrive from social interaction and structured activities.
Our programs are designed to cater to the unique needs of adults with developmental delays, focusing on enhancing their quality of life through structured activities and social opportunities. We ensure that all participants have a safe and nurturing experience. We cannot accommodate individuals who exhibit aggressive behavior.
To enroll your loved one in our adult day care program, we invite you to take the following steps:

Reach out to us at info@daydreams-ADCC to express your interest and ask any questions you may have.
Initial Assessment:
Schedule an appointment for an initial assessment with our care coordinator to discuss your loved one's needs and determine if our center is the right fit.
Documentation:
Complete the necessary enrollment forms, which include medical history, emergency contacts, and consent for services.
Medical Clearance:
Obtain a physician's clearance that confirms the need for adult day care services and outlines any specific care requirements.
Individualized Service Plan:
Work with our care team to develop an individualized service plan that outlines the goals, activities, and support services tailored to your loved one's needs and preferences.
Trial Enrollment:
We recommend a trial visit to ensure that your loved one feels comfortable and enjoys the activities and environment at our center.
FINAL ENROLLMENT:
Once all documentation is received, the individualized service plan is in place, and the trial visit is successful, we will finalize enrollment and set a start date for attendance.
Fees:
If you have a Medicaid waiver DMAS or CSB may pay your fees. Otherwise, our fees are based on the level of care required and the number of days attended per week.
